Case Management
It is possible to remain living in your own home with some help but as there are so many services available it can be daunting to know where to start.
This is where case managers can assist you. Case managers will work with you and your family or carer to determine what your care needs are and develop and implement an individualised care plan.
If you are eligible, Care Your Way case managers can assist you through your Government funded Home Care Package, to source services that meet your needs.
However, if you do not have a Home Care Package or have complex and multiple needs Care Your Way can provide services privately.
Support and coordination can be provided by our case managers to enable you to have your care needs met. Care Your Way case managers will liaise with medical and other allied health professionals, be an advocate for you to ensure your rights are protected. You employ Care Your Way according to your needs; whether it be for short-term or as an ongoing support.
When would I need a private case manager?
If you have little or no support from either family members. loved ones or other means you may choose to use a private case manager to organise your care. An advantage of using Care Your Way case managers is that we can provide assistance immediately. This means that your care can be in place when you want as there are no waiting periods for private case management compared to some government-subsidised packages – which is an advantage.
If you are returning home from hospital or recovering from an illness, Care Your Way case managers can liaise with the hospital transitional care team and have services in place immediately.
Our case managers can work with the Public Trustee and or Adult Guardian in situations where a person may not be able to make decisions for themselves and have no other family member or representative to assist them.
